骨科医学论文-闭合复位交锁髓内钉治疗胫骨干骨折
【摘要】 目的 观察闭合复位交锁髓内钉治疗胫骨干骨折的疗效。方法 回顾分析采用闭合复位交锁髓内钉内固定治疗的胫骨干骨折患者32例。结果 所有患者获得平均12个月( 5～24个月)随访, 均达到骨性愈合,平均临床愈合时间4.8个月。其中骨折延迟愈合4例，采用动力化后骨折愈合；发生骨不连2例,行二次手术植骨达骨性愈合。所有患者无血管神经损伤,未发生感染。疗效根据JohnerWruhs评定:优24例,良5例,可2例,差1 例,优良率90.6%。结论 闭合复位交锁髓内钉治疗胫骨干骨折损伤小、固定牢靠、疗效肯定,对于骨折延迟愈合患者动力化可加快愈合。

Therapeutic effect of closed reduction and interlocking intramedullary nails on fracture of shaft of tibia DENG Songbo, ZHANG Yaoqiang, XU Huorong. Huidong County People’s Hospital, Guangdong, Huidong 516300, China 【Abstract】 Objective To observe the therapeutic effect of closed reduction and interlocking intramedullary nails on fracture of shaft of tibia. Methods the data of 32 patients with fracture of shaft of tibia treated by closed reduction and interlocking intramedullary nails were retrospectively analyzed.Results All the cases were followed up for average 12 months (5～24 months), and all the patients got bone union. The mean clinical union time was 4.8 months in which 4 cases got bone fracture delayed union. After using postmotivation fracture union,2 cases was not healed, after the second operation with bone graft, the 2 cases got bone union. None of case occurred nervi vasorum injury or infection. According to the JohnerWruhs function scale,24 cases were excellent, 5 cases were good, 2 cases were acceptable and 1 case was bad, and excellent and good rates reached 90.6% .Conclusion The application of closed reduction and interlocking intramedullary nails in the treatment of fracture of shaft of tibia has advantages of little injury, stable fixation, definite therapeutic effect. Interlocking intramedullary nails can also speed up union in patient with bone fracture delayed union. www.med126.com

交锁髓内钉治疗胫骨干骨折近年来开展较多，具有可解剖复位、固定牢固的优点。是目前治疗胫骨干骨折的首选方法。本文选取自2003年3月至2008年3月我院采用闭合复位交锁髓内钉内固定治疗的胫骨干骨折患者32例进行回顾分析，临床效果满意，报告如下。
