医学免费论文:血清Galectin-3对胰腺癌诊断价值的初探
【摘要】 目的：探讨联合检测血清半乳糖血凝素-3(galectin-3)水平、癌胚抗原(Carcinoembryonic Antigen，CEA)和糖链抗原199(Carbohydrate Antigen 199，CA199)对胰腺癌的互补诊断价值。方法：ELISA法检测胰腺癌30例、胃肠道肿瘤18例、胰腺炎13例、正常人16例血清galectin-3浓度，同步测定血清CA199及CEA，比较galectin-3与CA199、CEA诊断胰腺癌的灵敏性及特异性， galectin-3与CA199和CEA对胰腺癌的互补诊断价值。结果：(1)血清galectin-3在胰腺癌患者的检出率为30.0%，而胃肠道肿瘤、胰腺炎、正常人检出率为0%;(2)ROC曲线分别确定galectin-3、CEA及CA199对良、恶性胰腺疾病的鉴别界值分别为0 ng/ml 、5.2 ng/ml 及35 U/ml。Galectin-3、CEA及CA199在胰腺癌中诊断阳性率分别为33.3%、50.0%及63.3%;(3)血清galectin-3与CEA及CA199间均无相关性 (P均>0.05)，联合检测galectin-3、CEA及CA199对胰腺癌诊断具有互补价值。结论： 胰腺癌患者血清Galectin-3升高, 联合检测Galectin-3与CA199和CEA可提高对胰腺癌的诊断敏感性。

The significance of serum galectin-3 in the diagnosis of pancreatic cancer

[Abstract] Objective: To study the serum galectin-3 level in the cases with pancreatic cancer and to explore the complementary value of galectin-3, CEA and CA199 in the diagnosis of pancreatic cancer. Methods: Serum galectin-3 were measured with ELISA in 30 cases with pancreatic cancer, 18 cases with gastrointestinal cancers, 13 cases with pancreatitis and 16 healthy persons. CA199 and CEA were measured with chemiluminescence immunoassay. The sensitivity and specificity of galectin-3, CA199 and CEA for diagnosis of pancreatic cancer were compared. The complementary value of galectin-3, CA199 and CEA for diagnosis of pancreatic cancer was also studied. Results：(1)The detection rate of galectin-3 was 30.0% in cases of pancreatic cancer, while 0% in cases with gastrointestinal cancers, pancreastitis and healthy persons. (2)Based on ROC curve, the cut-off value of galectin-3, CEA and CA199 for differentiating benign and malignant pancreatic diseases were 0 ng/ml, 5.2 ng/ml and 35 U/ml respectively. The diagnostic positive rates of galectin-3, CEA and CA199 for pancreatic cancer were 33.3%, 50.0% and 63.3% respectively. (3)Galectin-3 does not correlated with CEA or CA199(P>0.05). Combined determination of Galectin-3, CEA and CA199 are complementary to the diagnosis of pancreatic cancer. Conclusions: Serum galectin-3 increases in the cases with pancreatic cancer. Combined determination of serum galectin-3, CA199 and CEA may increase the diagnostic sensitivity for pancreatic cancer.

半乳糖血凝素-3(galectin-3)是半乳凝素家族的一员，是能识别糖蛋白和糖脂的特异性低聚糖结构。galectin-3在某些肿瘤组织中高表达[1-3]，其在肿瘤的发生与发展过程中所起的作用正日益受到重视。我们曾通过双向电泳及MALDI-TOP-TOP质谱分析技术，对胰腺癌组织和正常胰腺组织蛋白质组进行对比分析，最终质谱鉴定得到胰腺癌的16个差异表达的蛋白质，其中galectin-3即为表达上调蛋白之一[4]。迄今为止，尚未见胰腺癌患者血清galectin-3的研究报道。本研究检测了良恶性胰腺疾病及其它恶性肿瘤患者血清galectin-3水平，研究血清galectin-3检测对胰腺癌的诊断价值及galectin-3与CEA与CA199诊断胰腺癌的互补诊断价值。
